Get Registration Form

There are now two ways to secure your registration:

• Register Online and receive a Registration Number.
  Send the DEPOSIT, with your registration number, to the address below.

OR


• Print the Registration Form and fill it out.
  Send the Registration Form, along with the DEPOSIT, to:

All required paperwork, along with any remaining balance due, must be submitted BY JULY 31, 2010:

1. Completed Registration Form with Parent Release/Authorization and Insurance Information:
Online registrants need not fill out a paper Registration Form, however the Parent Release/Autorization and Insurance information from the back of the paper Registration Form is still required and may be downloaded from the website. Campers who leave out summer mileage and personal best times information from the Registration Form will be placed in the slowest running group at camp. 2. Stamped/signed School or Physician's physical of camper:
Physicals must be dated after August 21, 2009 and include record of allergies and immunization history. A wellness letter from your physician will also suffice. (RunningWorks does not provide physical form)

3. Parent and Camper signed Camper Agreement:

CAMP COSTS

Camp Canadensis August 16th through 21st
Total Tuition: $525 Deposit: $325

Full payment or a deposit of $325 is due immediately to secure registration.
Any balance remaining is due by July 31, 2010

Tuition includes all meals, camp t-shirt, water bottle and goodies from our sponsors.

Deposit is non-refundable.

Tuition Rebate: A $10 cash discount will be given ONLY AFTER CHECK IN AT CAMP, if 5 or more people attend from a girl's team or 5 or more people attend from a boy's team.
