ONLINE REGISTRATION

  1. Fill out the following form and submit your information.
    You will be given a registration number.
  2. Send the non-refundable deposit, with your registration number, to:

    Deposit = $325
  3. Print out the Parent Release/Authorization Form and fill it out
    AND
    Get a copy of your School Physical/Dr's Physical.
    Physical must be dated after August 21, 2009 to be valid.
    A wellness letter from your physician will also suffice.
  4. All Documents, along with the Remaining Balance Due,
    must be submitted NO LATER THAN JULY 31, 2010.
